New Patient Forms

All new patients (and/or parents/legal guardians of patient) are required to fill out our enrollment form and sign our policy agreement before Dr. Martin can see them. You can find links to both forms below.

Enrollment Form

The New Patient Enrollment form will be used for history and diagnostics during your one hour evaluation with Dr. Martin. You can fill out and submit the form by clicking the button with the appropriate age group below, but don’t forget to come back and sign the policy agreement!

Policy Agreement

You must sign our policy agreement before Dr. Martin can evaluate you. If it is for a patient under the age of 18, the patient’s parent or legal guardian will need to sign. To sign that form and submit it, click the link below.
