Returning Patients
READ CAREFULLY***
To be considered a Returning Patient, you must have had an appointment with Dr. Martin within the last 5 years. If it has been over 5 years since your last appointment, please schedule a New Patient appointment (below).
If you have had an appointment with Dr. Martin within the last 2 years, schedule a Med Check appointment. (Skip ahead and click “Returning Patients” button below.)
If it has been over 2 years and less than 5 years, you are required to schedule a Re-Evaluation appointment. This gives Dr. Martin time to catch up with you. (You will also need to fill out the “new patient” questionnaire packet again. To get that, click the “Forms” link above.)

New Patients
Your first appointment will be a complete Psychiatric Evaluation. This procedure allows Dr. Martin to render an accurate diagnosis and recommend a treatment plan. Click the button below to get started.
