Procedure for New Patient Appointments

READ THIS FIRST

Here is a brief overview of how to set up your first appointment with Dr. Martin. This side of the page will remain static so you can easily refer back to it.

1. Enroll in our Online Scheduler on the right side of this page. After completely filling out the form, we highly recommend that you write down your username and password. You will need that info every time you login.  When you are finished, click on the "Enroll" button.

2. Now click the "Make Appointment for Psychiatric Evaluation" button on the right side and pick an appointment time.

IMPORTANT!

3. After setting your appointment, click on the "Get Forms" button below. Print the required new patient forms, fill them out completely and bring them to your appointment.

Please call our office within 48 hours to verify and confirm your first appointment with Dr. Martin is scheduled correctly.
